Appointments
Office hours are Monday - Friday 8:30am - 4:30pm, closed daily for lunch from 12:00pm - 1:00pm. Patients are seen by appointment only.
Appointments can be scheduled by calling the office during normal business hours at (208) 535-4567.
New patients are asked to arrive 20 minutes early to complete paperwork, or print out the forms below & complete paperwork and bring the forms and your insurance card(s) with you to your appointment.
Cancellations
To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments.
